SERVICES and OPERATIONS PLANNINGNavy & Marine Official Request ProceduresClick here to download, fill out, save, and send in an Official Request Form (Excel Spreadsheet) After reading the general guidance below, please use the link at the top or bottom of the page to download, fill out, save, and send in an Official Request Form. The process is very easy and should take no more than a few minutes to do. Fill out the excel spreadsheet with the detail as you know it now. It is not by any means crucial to have the exact details of the desired mission/missions known. The primary day/days are usually sufficient. In the description of events, put a description of the event that is going to be supported and how the tanker will support that event. If the training areas that will be obtained by the receivers is known, indicate that in the text. If the tanker is a go/no-go criteria, indicate that as well. If part of your pre-deployment work-up schedule, indicate how this request supports that. If utilizing the tanker will make your training either more realistic, more efficient, and/or more effective, indicate how. The purpose of this detail is to provide the funding justification for the mission and to be able to prioritize the mission in case there is a scheduling conflict. If the mission requested is a cross-country drag, indicate the reason for the transit- i.e. to what training event or deployment are you going such as "Squadron deployment to/from Fallon for SFARP." When in doubt, more detail is better, if known. Rather than just saying, "Supporting LFE training," indicate the complexity of the events, how many units are participating, what the training goals are, as well as listing any logistical restrictions. If either the primary or one of the secondary missions is to obtain tanker qualifications for a certain reason, indicate that as well. If requesting unit level training support, specifically identify the desired categories of training and how using the tanker will make the training either more effective or efficient. When complete, save the form with your squadron name and the date of the request for the tanker. This allows for easier administrative processing. Then press the 'Submit' button on the bottom of the form. This will open up a new blank email on your screen with 2 automatically included email addresses. One of these is the Navy Commercial Air Services (CAS) Flight Hour Manager and the other is Omega Operations account. The Subject line of the email will reflect the title of your request. Attach the form to the email and then press send. No text in the email itself is required. Click here to download, fill out, save, and send in an Official Request Form (Excel Spreadsheet) |
